Affordable Dental Plans with Big Business Benefits

dentistBEST Life’s Dental PPO and Indemnity products are available for groups as small as 2 employees enrolling. Both products offer dental plans as employer-contributory or voluntary. Employers can choose from a selection of national or regional networks, and all dental include a supplemental dental accident benefit.

  4. Topaz Dental (Indemnity and PPO) - Utah Only

 

BEST Life PPO Dental

These dental plans offer the right blend of choice and cost-savings features. Employers may choose from a set of basic, mid or rich plan designs. Child and adult orthodontia are also available. Implants and posterior composites are standard, regardless of group size.

PPO Dental plans have in-network incentives (i.e. higher in-network benefits) and offer members greater cost-savings when services are provided by a network dentist. Members are free to choose a non-network provider or a dentist from a regional and/or national network. For Network Information click here.

This product is available in AZ, CA, DC, FL, IL, IN, MD, MI, MO, NE, NV, OH and PA.

Voluntary Dental Plans

If your group cannot meet the participation requirements of our employer-contributory plans, it may qualify for the voluntary option of our dental products. Our voluntary plans only require a minimum of five employees enrolling and 20% participation.

In addition to offering the same plan designs as our employer-contributory plans, voluntary plans allow employers the opportunity to offer a low-cost dental package to employees at no additional cost to the employer.

Special Offer (Valid for 2012 Effective Dates)

Offer voluntary groups lower rates! Dental voluntary groups who demonstrate above 60% participation can get their dental plan at employer-contributory rates. To use this incentive, present both voluntary and employer-contributory rates to your clients. Let them know that if they get above 60% participation, BEST Life will honor the employer-contributory rates. When you are ready to submit a new case, include the employer-contributory rates and make sure the employer application indicates that this is a voluntary case.

Underwriting reserves the right to apply voluntary rates if participation is not above 60%, so please make sure that there is indeed above 60% participation.

IndemnityPlus Dental

IndemnityPlus is BEST Life's indemnity plan that offers additional access to a PPO network. The same benefit levels apply in or out-of-network and members can select any US licensed dentist. They may also choose to save on out-of-pocket costs when they receive care from a PPO network provider.

Texas pPO Dental and IndemnityPlus Plans

These dental plans offer the right blend of choice and cost-savings features. Employers may choose from a set of indemnity or PPO-MAC plans to fit their needs.

Our Texas PPO Dental plans are designed for high network usage, and provide members with access to two PPO networks, CONNECTION Dental and DenteMax. Services received from a non-network provider are based on a set fee schedule.

Topaz Dental Plans - for Utah Only

These dental plans offer the right blend of choice and cost-savings features. Employers may choose from a selection of indemnity or PPO plans to fit their needs.

Topaz DPPO plans have in-network incentives (i.e. higher in-network benefits) and offer members greater cost-savings when services are provided by a Total Dental Administrators ("TDA") network dentist.

Topaz dental indemnity plans offer the same in and out-of-network benefits and give members the freedom to choose any dental provider in the country. Members may also access the TDA PPO network to save on out-of-pocket costs.

Both plans offer employers a selection of basic, mid or rich plan designs. Child and adult orthodontia are optional and implants and posterior composites are standard, regardless of group size.

These plans are available in UT only.
