BEST Life has two medical plans designed to fit the needs of businesses with 2 to 50 employees enrolling: a traditional PPO medical plan, and a HSA-compatible High Deductible Health Plan.
Both plans include Pre-certification services, and prescription coverage through Caremark. Employers may select from regional and national networks in AZ, GA, ID, IL, IN, MO, NV, OH, OK, TN, TX and UT.
Health Solutions II
For groups who like traditional PPO medical plans, our “Build Your Own” format gives them the choice to select the options they want:
- Broad range of deductible, co-pay, coinsurance and out-of-pocket maximum combinations;
- Preventive care services covered at 100%
- Prescription coverage with the choice of:
- 3-tier (generic/brand/non-formulary) co-pay options alongside Rx deductible options,
- 50% co-pay only (no deductible), or
- Discount Only Card (no co-pays or deductible)
Prescription Coverage
Health solutions II include prescription benefits or discounts from CVS Caremark. CVS Caremark’s network has over 60,000 participating pharmacies nationwide and offers the option to refill prescriptions by mail. A CVS Caremark Prescription or Discount Card is included along with the member’s ID card and Certificate of Insurance. Our High Deductible Health Plan (HDHP) can be enjoyed alone or it can be paired with a Health Savings Account for significant tax savings. HDHP members can use any bank to open a Health Savings Account and can take advantage of tax benefits, ease of payment, and online account administration.

Prescription Coverage
Members on our High Deductible Health Plan will receive a CVS Caremark Discount Only Card. Benefits include the option to refill prescriptions by mail and to receive discounted prices from a CVS Caremark pharmacy. There are no co-pays, and costs for prescriptions are applied to the High Deductible Health Plan’s deductible and then to the coinsurance.

Pre-certification Services
BEST Life's health plans include cost-savings tools that benefit both the employer and the member.
Unnecessary or excessive medical costs are a waste that no individual or company can afford. With the increasing cost of hospitalization and health care, excessive medical costs can be avoided by simply remaining alert to proper utilization. BEST Life's Pre-certification services are required on our medical plans. Most of the time, doctors will file a Pre-certification on a member's behalf, but members can also call the phone number provided on the back of their ID cards to talk to a Registered Nurse and get a Pre-certification.

Vision Discount Programs
As BEST Employer Association members, all
BEST Life members are automatically eligible to receive discounts for the following BEA sponsored vision discount programs. Other discount programs are available from BEA's website at www.beassoc.org/benefits.html.
Members will receive an explanation flyer and a Summary of Discounts along with their BEST Life ID card. A BEST Life ID card will be necessary to obtain discounts.
EyeMed Discount Program
Discounts for exams, glasses, lens options and add-ons, contact lens and even laser vision correction are included with no limitations on frequency. Members can obtain these discounts through EyeMed’s network, which includes many leading optical retailers including LensCrafters®, Target Optical®, Sears Opticalsm, JC Penny® Optical and most Pearle Vision locations.
QualSight Laser Eye Surgery Discounts
Receive 40 - 50% off the national average charge for laser eye surgery procedures administered by QualSight. Members should call 1-877-507-4448 before seeing any LASIK provider in order to obtain this discount. More information is located below and on QualSight's website.
Medical Network Availability
BEST Life has contracted with regional and national PPO networks to provide our members the best network coverage for their area.
BEST Life members who do not have PHCS/Multi Plan as their primary PPO network automatically get access to the Healthy Directions PHCS Network. This provides members with in-network benefits even when they are outside their primary PPO service area!
