Group Short Term Disability:
A Safety Net for Valued Employees

healthBEST Life's Short Term Disability plans help valued employees maintain their standard of living as they recover from non-work-related injuries or illness.

We offer four flexible plans to meet a variety of employer group scenarios. Available only in AR, DC, HI, ID, IL, IN, KY, MO, MS, NE, NM, OH, PA, SC, SD, TX, UT and WY.

100% employer-Paid Plans

Available to groups with a minimum of 5 employees enrolling. These plans are covered entirely by the employer and require 100% participation.

  1. No evidence of insurability required;
  2. No late entrants;
  3. Benefits may be based on a percentage or flat dollar amount up to $1,000 weekly for preferred industries;
  4. 13 or 26 week benefit duration;
  5. A choice to have benefits for a sickness/injury to begin in 1/8, 8/8, 15/15 or 30/30 days;
  6. Partial Disability and Maternity optional.

Employer-Contributory

For employers who want to contribute less than 100%, we offer this plan with a 25% minimum employer contribution.

  1. Requires 75% participation and a minimum of 5 employees enrolling;
  2. Percentage or flat dollar amounts up to $1,000 weekly for preferred industries;
  3. 13 or 26 week benefit duration;
  4. A choice to have benefits for a sickness/injury to begin in 1/8, 8/8, 15/15 or 30/30 days;
  5. The option to add coverage for Partial Disability or Maternity .

Voluntary

Designed for groups with 5 or more employees enrolling. This is a perfect option for companies looking for another benefit to add to their employee benefits program, without adding to their budget.

  1. Requires 25% participation;
  2. Benefits paid at a percentage or flat dollar amount up to $1,000 weekly for preferred industries;
  3. 13, 26, or 52 week benefit duration options;
  4. A choice to have benefits for a sickness/injury to begin in 1/8, 8/8, 15/15 or 30/30 days;
  5. Coverage for maternity optional.

Custom Plans

Groups with over 50 employees enrolling may opt to customize any of our Short Term Disability plans.

  1. May be offered as 100% employer-paid, employer-contributory or voluntary;
  2. Benefits paid at a percentage of the employee's salary or at a flat dollar amount up to $2, 500 weekly for preferred industries;
  3. May choose from 13, 26 or 52 week benefit duration periods;
  4. A choice to have benefits for a sickness/injury to begin in 1/8, 8/8, 15/15 or 30/30 days;
  5. Partial Disability and Maternity optional
