BEST Life Access Vision (Indemnity)

1. Available for groups with 5 or more employees enrolled
2. Employer-contributory and voluntary available
3. No ineligible industries, industry loads or waiting periods
4. Access care from any licensed ophthalmologist or optometrist
5. Contacts in lieu of OR in addition to frames and lenses
6. Medically necessary contact lenses included as eligible expenses
7. Flexible frequency, allowance and deductible options
8. EyeMed Vision Discount program for Access Vision plan members, includes discounts for exams, glasses, lens options and add-ons, contact lenses and even laser vision correction are included with no limitations on frequency. Members can obtain these discounts through EyeMed's network.

BEST Life Vision PPO Plan

1. Available for groups with 5 or more employees enrolled
2. Network access to over 50,000 providers nationally through EyeMed's Access Network
3. Contact lenses in lieu of eyeglass lenses
4. Plan allows member to receive either contacts and frame, OR frame and eyeglass lens services.
5. Access to care includes optometrists, ophthalmologists, opticians, private practices and many leading optical retailers including LensCrafters®, Target Optical®, Sears Opticalsm, JC Penny® Optical and Pearle Vision locations
6. 15% off retail price for Laser Vision Correction included
7. Materials Only plans available upon request

Laser Eye Surgery Discounts¹
