BEST Life has two medical plans designed to fit the needs of businesses with 2 to 50 employees enrolling: a traditional PPO medical plan, and a HSA-compatible High Deductible Health Plan.
Both plans include prescription coverage through CareMark, and employers may select from regional and national networks in AZ, GA, ID, IL, IN, MO, NV, OH, OK, TN, TX and UT.
Health Solutions II
For groups who like traditional PPO medical plans, our “Build Your Own” format gives them the choice to select the options they want:
- Broad range of deductible, co-pay, coinsurance and out-of-pocket maximum combinations;
- Preventive care benefits with buy-up options available
- $250, $500 or $1,000
- Prescription coverage with the choice of:
- 3-tier (generic/brand/non-formulary) co-pay options alongside Rx deductible options,
- 50% co-pay only (no deductible), or
- Discount Only Card (no co-pays or deductible)
Health solutions II include prescription benefits or discounts from CareMark. CareMark’s network has over 60,000 participating pharmacies nationwide and offers the option to refill prescriptions by mail. A CareMark Prescription or Discount Card is included along with the member’s ID card and Certificate Booklet. To find out more about CareMark, you can visit their website at www.caremark.com.

Health Solutions - High Deductible Health Plans
Our High Deductible Health Plan (HDHP) can be enjoyed alone or it can be paired with a Health Savings Account for significant tax savings. HDHP members can use any bank to open a Health Savings Account and can take advantage of tax benefits, ease of payment, and online account administration.

*Employer groups and members can open an HSA with any bank. If your client does not have an HSA provider, your BEST Life Sales Representative or General Agent can recommend one.
Members on our High Deductible Health Plan will receive a CareMark Discount Only Card. Benefits include the option to refill prescriptions by mail and to receive discounted prices from a CareMark pharmacy. There are no co-pays, and costs for prescriptions are applied to the High Deductible Health Plan’s deductible and then to the coinsurance.

Discounts on Vision
Starting with 2009 effective dates, BEST Life is offering qualifying Health plan participants a Vision Discount program through EyeMed.
Discounts for exams, glasses, lens options and add-ons, contact lenses and even laser vision correction are included with no limitations on frequency. Members can obtain these discounts through EyeMed's network; a BEST Life ID card will be necessary to obtain discounts.
Qualifying health plans include HSII groups who have multiple lines of BEST Life coverage and all HDHP groups.
Medical Network Availability
BEST Life has contracted with regional and national PPO networks to provide our members the best network coverage for their area.
BEST Life members who do not have PHCS/Multi Plan as their primary PPO network automatically get access to the wraparound PHCS/Multi Plan network. This provides members with in-network benefits even when they are outside their primary PPO service area!
